Geriatric Support Services


Geriatric Support Services provide case management services by, or under the supervision of, a Geriatric Mental Health Specialist to assist clients in accessing community resources and supports to live as independently as possible.

Below is a sample of services that may be provided:


Outreach & Engagement - Meeting with the client off site to provide case management and medication management.


Transportation Assistance - Case managers educate clients about transportation options and how to access them.  In some cases, transportation is provided by the clinician or case aide.


Family Support & Education - Case managers work with family and friends whom the client identifies to educate them as to the symptoms of the client's diagnosis and ways in which family and friends can assist the client.  May be provided in family or group meetings.


Medication Management - Case managers and medical staff work together to provide clients with safe access to medications.  This may mean: assisting the client with setting up a med box; distributing medication on a scheduled basis; advocating for the client with pharmaceutical companies who supply medication free of charge to the client; regular visits with the nurse or doctor; follow-up care and consultation with the client's personal physician, etc.


Hospital Diversion - Case management works closely with those clients at risk for psychiatric hospitalization by providing in-home stabilization services or accessing placement in the Crisis Respite/Hospital Diversion House.
