Community Support Services
Community Support Services provides case management services to assist clients in accessing community resources and building the skills needed to live as independently as possible.
Below is a sample of services that may be provided: |
Outreach & Engagement - Meeting with the client off site to provide case management and medication management. |
Housing Support & Advocacy - Clinicians work with clients to find and maintain safe, affordable housing. |
Transportation Assistance - Case managers educate clients about transportation options and how to access them. In some cases, transportation is provided by the clinician or case aide. |
Family Support & Education - Case managers work with family and friends whom the client identifies, to educate them as to the symptoms of the client's diagnosis, and also teaches ways in which family and friends can assist the client. May be provided in family or group meetings. |
Protective Payee - Often, DSHS requires the client to have a protective payee. In these circumstances, CMHC receives and disperses the client's income in order to assist in meeting day to day and monthly bills. The goal is to educate the client on budgeting skills in order to return control of the income to the client. |
Medication Management - Case managers and medical staff work together to provide clients with safe access to medications. This may mean: assisting the client with setting up a med box; distributing medication on a scheduled basis; advocating for the client with pharmaceutical companies who supply medication free of charge to the client; regular visits with the nurse or doctor; etc. |
Hospital Diversion - Case management works closely with those clients at risk for psychiatric hospitalization by providing in-home stabilization services or accessing placement in the Crisis Respite/Hospital Diversion House. |
